Request a replacement certificate

    If you have trained with me and lost your certificate, you may request one via this form. All information is required. This form can only be used for trainings conducted by Kari Kwinn, not YTT programs. Upon completion of this form, you will receive an invoice to pay for your replacement. Electronic certificates will be re-issued within one week, paper may take longer depending on travel.
